Yoga for People with Medical Conditions

Carol Krucoff’s post last week Yoga May Be the Best Activity for the Elderly inspired me to interview Shari Ser about the Yoga for Health class at the Berkeley Yoga Room that she and Bonnie Maeda, RN teach on a weekly basis. This class is special because it’s for seniors with medical conditions as well as general aging dysfunction, and current students include those bilateral total knee replacement, severe osteoporosis, coronary artery bypass graft and other heart issues, stroke, vertigo related conditions, severe arthritis, fibromyalgia, and several hip fractures.

Nina: Why did you two decide to start your Yoga for Health class

Shari: We started this class because we wanted a safe venue to offer to seniors who couldn't participate in a regular gentle yoga class or a Yoga for Seniors class due to individual medical conditions that might make it more challenging for them to safely participate. We intentionally organized the class so there would always be two trained teachers present at all times. One teacher primarily teaches and the supporting teacher assists students who might need modifications other than the way the pose was being taught.

Nina: How is yoga helpful for people with medical conditions?

Shari: Yoga is helpful for all that ails you! It builds strength, stamina, and balance. It also builds confidence and the mental equanimity that you can face anything. The fact that yoga is portable and can be done anywhere is also extremely attractive. You can do yoga in bed, sitting in a chair, driving in a car. Your mat doesn't have to be confined to a physical space but your practice situation can truly be anywhere.

The ability to face any critical illness is aided by stress reduction techniques that Baxter, Timothy and Nina have addressed in previous posts. Being able to monitor your breathing allows you the mental clarity to think things through in a more methodical and calm manner. Learning to find your own focus point whether it is learning how to stand on your toes or dealing with obtuse health insurance adjusters when contesting a claims bill is equally important. I can't overemphasize the importance of the relaxation response when living with chronic health issues.

Furthermore, for so many people their preconceived notions of what they can or can't do is astonishing. People very often tend to underestimate their abilities because no one has ever taken the time to help them break down a series of movements into bite size pieces that they can and do hungrily consume. The look of joy and astonishment on a student's face when they can successfully participate in class continues to be both Bonnie's and my guiding light in our teaching!

But people who are living with chronic medical conditions apart from general aging do present particular unique challenges for the yoga teacher. You literally are thinking on your feet at all times during class because modifications need to be individualized and often times that may precipitate a lot of prop hauling to get the asana just right for the student to begin to reap the healing benefits. An extra set of hands or two is invaluable!

Nina: I love the idea that you help people learn they are capable of more than they think—that's good for the spirit as well as the body. So, what are your classes like?

Shari: All of our classes work on the entire body as well as encouraging a practice space that feels safe and nurturing. It is our circle of practice that invites the student to join us. Also, we try to adequately challenge the students within their abilities. So every class has a balance component with lots of strengthening. We work a lot on developing core stability with flexibility in our shoulders, hips, spine, knees and ankles. Working on upper body as well as lower body strength is also critically important. We also talk about the carryover in their own lives, like being able to turn their heads when they are changing lanes if they still drive, reaching for the item overhead in their kitchen, or as mundane as being able to tie their own shoes.

Nina: I think we'd all agree that helping people stay independent as they age is absolutely invaluable! What should people with medical conditions consider when they look for and/or start to take a yoga class, or start practicing on their own at home?

Shari: Certain medical conditions may warrant formal physician clearance for participation in a yoga class. Our personal opinion is all recent surgeries need official clearance. Other than that it is the teacher's responsibility to keep the student safe but it is equally important that you, as a student, pay attention to how you are feeling during class and to inform the teacher if you notice any sudden changes in yourself during class.

We also think as a student you should ask the prospective teacher specifically what their training is, are they experienced with dealing with your medical condition and can you stop as needed during the class. Also can you decline to participate in an activity that you do not feel ready to do? This is an important question because it tells you how adaptable the teacher will be if special circumstances arise during class.

You should also consider the size of the class. We think a class needs to be small, with no more than 10-12 with two teachers, because this type of population needs close monitoring and frequent rest and hydration breaks. Lastly, the class needs to be fun! Joking and having people share with each other builds a sense of community among the students.

An interesting potpourri of patents were issued to companies and individuals involved in the medical laser industry over the past 18 months. As the industry becomes more application oriented, more patents seem to be aimed at meeting the needs of the identified niches. In a rough count of the 50 plus patents reviewed, 4 were involved with delivery catheters for angioplasty; 13 were directed at dental applications, including the use of dual wavelengths to treat both hard and soft tissue and special handpieces for hard to reach areas of the teeth; about a dozen were aimed at corneal sculpting -- or the alleviation of some of its problems; more than a dozen concerned catheters for delivering laser energy within the body -- either directly at a tissue or at some divergent angle; several patents dealt with multiple wavelength outputs, control of laser pulses or dosimetry; and another half a dozen or so involved better control of the lasing process itself.

Here then is a sampling of some of the more interesting medical laser patents issued last year and for the first several months of 1994.

Advanced Interventional Systems

● US 5,188,632 (2/23/93) -- A waveguide for ablating lesions in blood vessels, incorporating an expanding lens at the distal end of the laser fiber to provide a larger ablation area -- one of a series of ablation catheter tips assigned to AIS.

American Dental Technologies (7 patents issued during the period)

● US 5,275,564 (1/4/94) -- A pulsed holmium dental laser for welding of enamel and hydroxyapatite.

● US 5,257,935 (11/2/93) -- A laser for removing dental enamel, dentin, and soft tissue using a wavelength between 1.5 and 3.5 microns.

● US 5,232,367 (8/3/93) -- A method for sterilizing or reducing bacteria in accessory canals or closing same with a pulsed laser output of between 0.1 mj and 5 joules per pulse.

● US 5,207,576 (5/4/93) -- A dual wavelength dental laser with one laser having a pulsed output between 0.2 to 2.0 microns, and the second pulsed laser having an output between 2.0 and 5.0 microns.

Candela Laser

● US 5,287,380 (2/15/94) -- A method of generating long pulses (greater than 500 ms) from a flashlamp-excited laser, using a ramp pulse having an amplitude increasing with time.

Intelligent Surgical Laser (2 patents issued in the period)

● US 5,246,435 (9/21/93) -- A method of using an ophthalmic laser to remove cataractous tissue from within the capsule.

Laser Medical Technology (6 patents issued during the period)

● US 5,292,253 (3/8/94) -- A method of repairing an opening in a bone or tooth by applying a laser beam to weld a calcium containing protein gel to the bone or tooth tissue without harming the bone or tooth tissue.

● US 5,290,274 (3/1/94) -- An apparatus comprising two laser sources operating at different wavelengths and power levels, capable of cutting a given organic tissue, simultaneously applying the two laser wavelengths to a single body region and directing a cooling fluid at the tissue while applying the radiation.

● US 5232,366 (8/3/93) -- A tunable laser capable of producing at least 2 wavelengths for use in the mouth for cutting physiological tissue, used in conjunction with a cooling fluid.

Laserscope (10 patents issued during the period)

● US 5,281,214 (1/25/94) -- A disposable surgical probe that can deliver laser energy either forward or deflected. When a curved optical fiber is retained within a sheath, the laser energy is directed forward; when the fiber is pushed out of the sheath, the fiber curves, deflecting the energy.

● US 5,257,991 (11/2/93) -- A side firing laser fiber having a beveled end contained within a cylindrical probe, having a cutout in the side of the probe.

● US 5,249,192 (9/28/93) -- A multiple frequency medical laser having outputs at 1.06, 1.44, and .532 microns.

● US 5,242,434 (9/7/93) -- An apparatus consisting of an elongated tube for inserting and guiding a laser beam for performing a discectomy of the herniated nucleus of a herniated disc.

● US 5,201,729 (4/13/93) -- Performing percutaneous discectomy with a 1.44 micron YAG laser.

● US 5,178,617 (1/12/93) -- A scanner system for distributing laser energy in a treatment pattern using a bundle of optical fibers, with a scanner configuration at the fiber input, and a treatment configuration at the output end.

Massachusetts Institute of Technology

● US 5,290,275 (3/1/94) -- Use of a transparent protective shield at the distal end of a multifiber catheter for mechanically displacing intravascular blood and protecting the fibers from the fluid. The shield also protects the patient in the event of fiber failure.

Premier Laser (4 patents issued during period)

● US 5,267,856 (12/7/93) -- A method of ablating a selected area of a material with a laser by adding a laser energy absorbable liquid to the area that enters the material via surface pores or is chemically bonded to the material. Application of the liquid is done either directly prior to or during each laser pulse, until desired amount of tissue/material is removed.

Summit Technology (4 patents issued during period)

● US 5,230,334 (7/27/93) -- The use of ultrasound for controlled localized hyperthermia, useful for inducing controlled collagen shrinkage in corneal tissue.

● US 5,214,071 (5/25/93) -- The use of topically applied aliphatic alcohols to reduce discomfort and inflammation, and to promote corneal wound healing and epithelial regrowth over a reprofiled corneal surface.

Sunrise Technology

● US 5,304,169 (4/19/94) -- A method for shrinkage of collagen by application of laser energy, wherein the threshold shrinkage temperature is substantially reduced by application of a reagent, such as lysozyme, to the tissue prior to heating. This allows the use of substantially lower energy levels.

Trimedyne (5 patents issued during period)

● US 5,242,438 (9/7/93) -- The means for deflecting laser energy circumferentially by reflecting the laser energy off of a cone placed in the path of the beam transmitted out of a fiber, within a catheter.

● US 5,242,437 (9/7/93) -- A medical device to apply heat to a site within a patient's body by using both irradiation and conduction to locally treat a tissue such as the endometrium. At the operative end of the device, some of the laser energy is absorbed, delivering heat omnidirectionally, while another portion is transmitted through an aperture for localized and intense heating and destruction of the target tissue.

MEDICAL ANALYSIS OF THE CRUCIFIXION AND DEATH OF JESUS CHRIST III


PILATE'S VERDICT


Mark 15:15 - "Wanting to satisfy the crowd, Pilate released Barabbas to them. He had Jesus flogged, and handed him over to be crucified."

The Sanhedrin met early the next morning and sentenced Him to death ( Matthew 27:1). Because the Jews were not, and the Romans were, able to carry out an execution, Jesus was brought before Pilate.

Then Pilate sends Jesus to Herod. Jesus is speechless before Herod, and Herod sends Him back to Pilate. Pilate is unable to convince the crowds of Jesus' innocence and orders Jesus to be put to death. But he ordered that Jesus be flogged first.

It is at this point that Jesus suffers a severe physical beating. During that kind of flogging, a victim was tied to a post, leaving his back entirely exposed. The Romans used a whip, called a flagrum or flagellum which consisted of small pieces of bone and metal attached to a number of leather strands.

The number of strikes is not recorded in the gospels. The number of blows in Jewish law was set in Deuteronomy 25:3 at forty, but later reduced to 39 to prevent excessive blows by a counting error.

The victim often died from the beating (39 hits were believed to bring the criminal to "one from death".) Roman law did not put any limits on the number of blows given.

During the flogging, the skin was stripped from the back, exposing a bloody mass of muscle and bone. Extreme blood loss occurred from this beating, weakening the victim. perhaps to the point of being unconscious.

Jesus' beating was so severe that He was beyond recognition. Isaiah 52:14: ". . . Just as there were many who were appalled at him - his appearance was so disfigured beyond that of any man and his form marred beyond human likeness".

              ROMAN SOLDIERS MOCK AND BEAT JESUS

Matthew 27:28-30 (The soldiers) stripped him and put a scarlet robe on him and then twisted together a crown of thorns and set it on his head. They put a staff in his right hand and knelt in front of him and mocked him. "Hail, king of the Jews!" they said. They spit on him, and took the staff and struck him on the head again and again.

Unlike the traditional crown which is depicted by an open ring, the actual crown of thorns may have covered the entire scalp. Some sources say the thorns may have been 1 to 2 inches long. The gospels state that the Roman soldiers continued to beat Jesus on the head. The blows would drive the thorns into the scalp (one of the most vascular areas of the body) and forehead, causing severe bleeding.

A Personal Journey How I Went From Being A Bench Chemist to An Expert Resource in Ophthalmology and Medical Lasers


A short while ago, I was asked by Maureen Duffy, editor of VisionAware, the blog of the American Foundation for the Blind, how I became so knowledgeable about ophthalmology and why I started my blog. I prepared some background information for Maureen and she published it as a guest blog on her site, but because of space limitations, she was only able to use an abridged version. Since I don’t have the same space limitations, I decided to publish the “unabridged” version here.

So, here is my story:

The Beginning of My Career in Chemistry

I graduated from the Univ. of Massachusetts in Amherst, MA in 1957 with a B.S. degree in organic chemistry. After three jobs in industry (The Refinery Technology Laboratory of Gulf Oil in Philadelphia – analyzing oils and gas products from the refinery; the Container & Sealant Lab at the Dewey & Almy Div. of WRGrace in Cambridge, MA – working on cap and can sealants for baby food and peanut butter jars and aerosol can valves; and The Exploratory Development Lab at United-Carr Inc. in Watertown, MA – working on adhesives to replace fasteners for automotive applications), I joined the staff of the Product Technology Section of Arthur D. Little (ADL) of Cambridge, MA, the international consulting firm, in March 1969, as a laboratory bench chemist.

Over my twenty-five years at ADL, I worked on hundreds of projects. Among the more memorial assignments were being part of the team that developed the “all plastic pencil” for the Empire Pencil Company (part of Hasbro Toy); working on the development of an erasable pen ink compound for Bic Pen; trying to produce a protective plastic liner for glass Thermos bottles (to protect the food contents of the bottles when school children hit their neighbors on the head with their lunch boxes – which proved a technical success – water, soup and coffee stayed hot overnight, but failed miserably, as the plastic liner took the flavor out of the coffee making it taste terrible!); developing a spin-cast epoxy eyeglass frame compound and manufacturing process for Universal Optical; developing an improved firefighter’s glove for NIOSH and NASA; the development of a unique, disposable, no moving-parts mixer for use in dispensing two-part epoxy and urethane adhesives for the MPB Corporation (now commonly used in all dentist’s offices for dispensing casting compounds and sold in hardware stores for delivering reactive adhesives and sealants); and of course being a part of the team that built and flew a lead balloon to prove that lead balloons really could fly! (In fact I tried to launch the silk purse made from sow’s ears in a basket beneath our lead balloon!)

I have written the stories behind many of the above inventions and products in my “other” blog, ADL Chronicles.

An Introduction to Soft Contact Lenses

One of my first assignments at ADL (along with working on chemically stabilizing soils for soft ground tunneling) was to see if there were other uses for hydroxy ethyl methacralate (HEMA), the material that soft contact lenses were composed of. National Patent Development Corp. (NPDC), which had acquired the Czech technology and licensed it to Baush & Lomb, wanted to know if this product could be used for other applications. We thought of applying it to the hull of boats to make them more slippery (and go faster in water and reduce adhesion of barnicles), but the best I could come up with was its use as a slow release reservoir for insecticides to kill mosquito larvae in swampy areas.

A few years later, in June 1972, and because of my knowledge of the properties of HEMA, NPDC asked me to lead a study on the “safety and efficacy” of the B&L Soflens, which had just been approved for marketing (March 1972). (NPDC was going to issue some stock to the public and the underwriters needed this due diligence report for the offering.) The study involved interviewing all of the original FDA clinical investigators for the Soflens. The knowledge about soft lenses I gained led to several papers about the “Outlook for Soft Lenses” that were published by ADL’s Decision Resource Division. Those publications led, over the next fifteen years, to over 150 assignments in the soft lens industry, including about 50 briefings to companies interested in both the technology and financial aspects of soft contact lenses. (How was B&L making so much money with their Soflens business?) It also led to my assisting Johnson&Johnson (Vistakon), Ciba Geigy (CibaVision), and Schering Plough (Wesley-Jessen) in their acquisitions of technology or small companies, to enter the soft lens industry. (In essence, I became the “guru” of the soft contact lens industry!)

Ophthalmology, Lasers and Writing

The soft lens assignments led to assessments of intraocular lenses (IOLs), plastic eyeglass lenses and eventually, in 1983, the just FDA-approved YAG ophthalmic lasers for correcting capsule opacities following cataract procedures. In 1985, I became involved with surgical lasers and in 1986 wrote the first comprehensive research and market report on medical lasers, “Medical Laser Systems: The Surgical Revolution”, that was published by Arthur D. Little’s Decision Resources Division. This was followed by a second comprehensive report on medical lasers, “The Outlook for Medical Lasers: The New Technologies”, published in 1988, also by Decision Resources. That led to my second career at ADL,  involving ophthalmic and medical laser consulting.

In the summer of 1985 I attended a Gordon Research Conference on lasers used with biological materials, where I met many of the leading researchers and surgeons involved in ophthalmic and medical laser technology. In December 1985, I was asked to prepare a market report about the potential for the excimer laser to correct vision. That report, about what I called LRK (Laser Photorefractive Keratectomy), became PRK, or ablation of the surface of the cornea to correct vision. My report was used to raise funds to begin one of the first excimer laser companies, Tauton Technologies (which later acquired VISX and assumed the VISX name).

That led to my entry into consulting in refractive lasers and writing the first market reports about refractive surgery in the mid-1980s and early 1990s. (I later wrote the seminal report on the future of laser refractive surgery for Summit Technoloy in 1992, wherein I predicted that the excimer laser would gain FDA marketing approval would occur by mid-1995. As I recall, I believe I was off by about 4 months.)

As I had begun attending ophthalmic and medical laser industry meetings (AAO, ASCRS, ARVO, ASLMS), I started writing about what I learned at those meetings. I first began writing columns for Vision Monday in 1988, mostly about my contact lens-related work. In 1989, I was asked to write a monthly column for Ophthalmology Management which became my Technology Update column. (Ophthalmology Management stopped publishing in the spring of 1991.) In the fall of 1991, Ocular Surgery News asked me to continue my Technology Update columns for them. This monthly feature, mostly covering new technologies that I discovered at the ophthalmic meetings, ran for over eleven year in OSN.

I was among the first to write about “custom ablation” and LASIK. One of my favorite titles from those times was, “Inlays, Onlays, Rings & Things” which described alternatives to laser refractive surgery.

Retirement and Spectrum Consulting

Finally, in March of 1994, on the anniversary of my 25th year with ADL, I retired from ADL and started my own consulting firm, Spectrum Consulting, continuing my consulting work in ophthalmics and medical lasers. In the fall of that year I started publishing Executive Laser Briefing, a monthly newsletter about current developments in the ophthalmic and medical laser industries. It was begun as an added service for my consulting clients, but I soon realized that others were interested in the publication. I began marketing the newsletter, which grew to a 40-60 page monthly publication, sent out to clients around the world, and it became a major part of my consulting business.

In December 2005, after eleven years, I  sold the newsletter and its client list to the publishers of Trends-In-Medicine, who continue to publish the newsletter today, renamed Executive Laser Report, and retired from active consulting.

A New Blogging Career Begins: Irv Arons’ Journal

That’s when I decided to begin writing my blog, Irv Arons’ Journal. It was originally started as a vehicle to place the more than 150 previously published articles online and accessible for historians and researchers, as I was involved in the beginning of several ophthalmic firsts – among them the birth of soft lenses, IOLs, ophthalmic lasers, refractive lasers, etc. Most of the articles were not available online because they were written prior to the explosion and availability of the web.

However, I quickly became diverted because I realized that I had insight into several new technologies that were entering ophthalmic use and I had access to those in the know who could provide me with the insight to write about these developments.

It all began because of some colleagues writing about what had happened the previous summer at the Retina 2005 Meeting in Montreal. Dr. Phil Rosenfeld presented information about his use of Avastin to stop the wet form of age-related macular degeneration (AMD) cold in its tracks and to provide improved vision to some patients. His presentation wowed the audience, but other than the retinal surgeons present at the meeting, no one else in the ophthalmic world was aware of the significance. I decided I needed to tell this story – Avastin: A New Hope for Treating AMD -- and so began my blogging career with an emphasis on new technologies for treating retinal diseases.

Since that start in December 2005, I now have over 270 articles posted online, mostly about new treatments for both dry and wet AMD, but also including writeups about new devices used to both treat and detect AMD.

Along the way, I have also written about the use of lasers to treat eye floaters, about the history and use of femtosecond lasers, including their use in treating cataracts, an overview of treatments for glaucoma, including the use of the then new SLT laser.

A Developing Interest in Stem Cells and Gene Therapy

Over the past several years, I became interested in the use, first of stem cells, and then gene therapy in the treatment of ophthalmic diseases. In September 2010 I wrote A Primer on the Use of Stem Cells in Ophthalmology, which was followed in November 2010 with my first writeup on the use of gene therapy in ophthalmology, The Use of Gene Therapy in Treating Retinitis Pigmentosa and Dry AMD. Since then, I have followed with more than 15 updates on stem cell treatments and more than a dozen for gene therapies.

In addition, I have put together comprehensive tables of the companies and institutions involved in both stem cells and gene therapies, the ophthalmic applications under evaluation, and data about the clinical trials underway and completed for both therapies. All of this information is now available online via my blog.
